Starting a Near Miss and Incident Report on App


  1. At the bottom menu, tap the hazard-icon icon
  2. Select Near Miss & Incident.
  3. Fill out the form as required.
    1. Date Occurred and Time Occurred fields will automatically default to the time and date the report was started.  To change these, tap in the required field and amend details as needed.
    2. Add Individual(s) involved by tapping the Add Individual button. Repeat to add multiple individuals. 
    3. (Optional) Upload images as required. Tap the camera icon-camera icon to upload a new image using your camera or browse to upload an existing image from your device by selecting Gallery.
    4. Add key witnesses by tapping the Add Witness button. Repeat to add multiple witnesses.
  4. Tap Save.
Note: Business Administrators and Supervisors will receive a push notification when a Near Miss & Incident report is started on the mobile application. 

App Notification - Business Administrators and Supervisors will receive a notification from the app with details of the Report Title, Reporter and Location notifying them a NM&I has been started that needs investigation.  There will also be a notification in your app via the bell.

Desktop Notification - Business Administrator's and Supervisors will receive a notification via the bell. Tapping on the notification will open the NM&I to complete the report.

Email Notification - ONLY Management Portal users connected to the relevant site will receive an email advising them of the NM&I Report:
